You do not apply directly. You mail a letter by REGISTERED mail requesting an application — Certified Mail is not accepted, and a separate letter is required for each application. Letters must not be postmarked earlier than October 19, 2026; the first 2,000 postmarked between October 19 and October 30, 2026 are mailed an application in January. A non-refundable $25.00 fee accompanies the application, waived for applicants receiving public assistance.
Testing frequency
